MatchCare Coordination

At the heart of our MatchCare Model is your care coordinator. Which is why we closely monitor our care coordinators’ caseloads to facilitate the personal connection between client and care team that leads to best at–home care possible. MatchCare Coordinators are available to clients and their families 24/7 so they can have help whenever they need it.

Care Specialists

Our MatchCare™ Model provides top-notch care because we have top-notch caregivers. Our team is composed of professional, experienced, and compassionate people-many of whom have specialized skills and expertise.

We put a lot of thought into pairing each client with a Caregiver who has the right experience and skill set for their needs. We want clients to have someone who will not only provide exceptional care but will make them feel comfortable and provide good company.

An important part of our MatchCare™ Model is having Caregivers who go beyond essential day-to-day care, but are committed to improving clients’ overall well being. We believe this is especially important for aging adults who need companionship and support for a happy, healthy lifestyle.

CONSISTENT CARE

Our MatchCare™ Model focuses on consistency, meaning each client will have either one Care Specialist or a consistent team of caregivers—depending on the type of care they need. If there is ever a change in caregiver(s)-due to illness, vacation leave, or for any reason-your Care Coordinator will introduce the new caregiver and familiarize them with the plan of care. This is one of the many ways we keep the number of people entering clients’ homes to a minimum, transitions smooth, and all parties involved in the care on the same page.

KC Shepherd’s Center: Offers services, comfort, and companionship to older adults in the greater Kansas City area. 

Clay County Senior Services: Serves North Kansas City and provides services for wellness, including home safety modifications for those 60 and older. 

KDADS (Kansas Department for Aging and Disability Services): Provides information, resources, and services for older adults and caregivers in Kansas. 

Area Agencies on Aging: These AAAs coordinate and offer services to help older adults remain in their homes, aided by services such as home-delivered meals, homemaker assistance, and more. 

Missouri Senior Resource Line (1-800-235-5503): Provides information and assistance on various services, including information and assistance, in-home services, transportation, legal assistance, nutrition, disease prevention, family caregiver support, and long-term care ombudsman services. 

Alzheimer’s Association: Heart of America Chapter in Prairie Village, KS. 

Missouri Department of Health and Senior Services: Provides information and resources, including guides on senior care, nursing homes, and long-term care facilities. 

Home Safety and Repair: Clay County Senior Services offers home safety repairs or modifications for eligible residents. 

Transportation: RideKC for transportation resources. 

Kansas Legal Services: Works with KDADS and AAAs to protect seniors’ rights. 

Mid-America Regional Council (MARC): Provides aging and adult services, with information and referral services. 

VA Caregiver Support Program: Promotes the health and well-being of family caregivers who care for veterans.
