At Private Home Care, we believe that every person should have the ability to transition from this life on their own terms.

So we work with clients and their families to create a customized plan for End of Life Care or Hospice support. We also thoughtfully match clients with experienced Caregivers who have the special skills to bring comfort to clients and their loved ones.

What is Hospice & Palliative Support?

Our hospice and palliative support services focus on assisting individuals who are nearing the end of life because they are terminally ill and expected to pass away, often within six months. This determination is reached with the assistance of a medical professional, such as a client’s primary care physician. Some of these clients are in hospice, while others are not. Our support is centered around palliative care, or the easement of the severity of pain or discomfort.

When hospice is in place, our care teams work closely to ensure that all directives from hospice staff are being followed, while providing between four and twenty-four hours of care per day.

Many of our clients choose to age in place and remain in a home setting until they pass. For others, Private Home Care is needed for assistance in the final stage of life. Either way, we’re here to help.

In what ways do Caregivers chosen for End of Life Care help their clients?

Like with all home health care services at Private Home Care, our approach to End of Life Care is always customized and client-focused. Caregivers selected to work with individuals in the final stage of life assist with personal care tasks like daily hygiene, toileting, grooming, bathing, dressing, and feeding.

They work together with a client’s loved ones, hospice teams, and others, and focus on easing each client’s feelings of stress and anxiety. Caregivers also are readily available to ensure their client’s loved ones are getting the respite and support they need, by assisting with meal preparation, light housekeeping, laundry, and errands. Our team’s goal is to provide clients and their families with the relief they need, so that the care recipient can end this life as comfortably as possible, on their own terms.

Does PHC offer specialized care that focuses on End of Life Care?

Yes. Not every home health care professional can provide end of life care. Working with a client in the final stage of life requires understanding garnered through experience and education.

Caregivers assigned to these clients know not only how to provide care, but also how and when to communicate with a client’s loved ones, a hospice team, third-party care managers, and the client’s Care Coordinator. They understand that in many situations, we are there for emotional support not only for the client, but also for their loved ones. Simply put, care teams and individual caregivers assigned to clients nearing the end of life are highly skilled and experienced in End of Life Care.
